We work to discover how the very large sugar molecule hyaluronan forms normal and abnormal environments that are required in every aspect of human development, from successful fertilization, to normal tissue changes during growth, to the protection and repair of tissues during many diseases and inflammations, and to cancer development.
Researchers in my laboratory work to determine the mechanisms for the aberrant synthesis of hyaluronan, a very large, negatively charged polysaccharide, in intracellular compartments when dividing cells are exposed to high glucose levels characteristic of uncontrolled diabetes. The mechanism was identified in kidney glomerular mesangial cells, and shows that cells entering division stop glucose uptake normally but cannot do so in high glucose levels. Intracellular hyaluronan in dividing cells has now been identified in hyperglycemic diabetic pathologies, and also in the current obesity epidemic brought on by the long-time adherence to the Western diet (low fat/high sugar).
We have also shown that low concentrations of heparin block glucose uptake within one hour in cells entering division in high glucose, and the cells sustain normal functions after division. Our ongoing research has shown that this mechanism is central to monocyte/macrophage pathologies. Monocyte progenitor cells dividing in high glucose produce circulating monocytes that contain intracellular hyaluronan, have a pro-inflammatory phenotype, and cannot successfully clear pathological hyaluronan matrices formed during inflammations. The presence of heparin prevents the intracellular hyaluronan response and allows the circulating monocyte to become a functional tissue repair anti-inflammatory phenotype in response to pathological inflammations.
